All City forms are now available in a fillable format. This means that you can fill out the information required by the form by typing in the information while the form is displayed on your computer monitor. You will not be able to alter, remove or add to any of the pre-printed text on the form. PLEASE NOTE: You cannot save data typed into these forms. Since the forms cannot be saved you will need to print the completed form.
